We strive to provide excellent care for our residents and will help you attain excellence in your career with us, so we have partnered with U.S. Enrollments to assist you.  An important part of your compensation package is the employee benefits made available to all eligible employees. Take the time to review the important information on this site so you can make informed choices when electing benefits.                  📌Eligible employees are benefits eligible on the first of the month following 60 days of full-time employment. 

You must enroll from your hire date until 1 day before your effective date, which is the first of the month following 60 days of employment. You can then enroll or decline any or all of the offerings.

        Example: If your SSN is 123-45-6789 and you were born in 1975, your PIN would be 678975

Scroll down or visit the Forms Library section at app.thebeaconselect.com/gulfreserve to review important benefit documentation for our plans, including the Benefit Guide, the Health and Welfare Plan Summary, the Medical Plan Summary, the Dental and Vision Plan summaries, and other plan documents and required notices. The forms library can be reached by clicking on the book icon in the upper-right corner of the screen.


Please note: The self-service site is available 24/7. You can modify your benefit elections only during Open Enrollment or if you experience a Life Event. However, you may update beneficiaries and download your benefit confirmation statement throughout the year.

📌New Hire Enrollment Timeframe: New Hires should enroll in the 1st half of the month prior to their effective date to ensure they avoid double deductions by enrolling too late and to ensure they have been added to carrier systems.
For Example, if your benefit effective date is 11/01, you should be enrolling in the 1st half of October.
Effective Date: 1st of the Month following 60 days.
NOTE: Any employee who enrolls within 7 days of their New Hire effective date may be subject to double deductions in their payroll check.
